The HOPE StudyHeart Outcomes Prevention Evaluation This is a great study that is just now making headlines even though it was published January 2000 in the New England Journal of Medicine. It was a large, randomized, double-blind trial with 256 centers participating and 9500 patients enrolled (which makes this a very powerful study due to this large number). The study evaluated whether an ACE inhibitor, ramipril, would prevent major vascular outcomes, specifically heart attack, stroke, or cardiovascular death, in those patients without heart failure. Patients included in the study were over 55 and either already had evidence of cardiovascular disease or had diabetes and one other risk factor for heart disease. Other medications that patients were already on were continued such as aspirin, cholesterol-lowering medications, and other blood pressure medicines. This study was stopped after 4 1/2 years as a 22% DECREASED risk in the occurrence of a heart attack, stroke, or death from cardiovascular disease was observed. A difference in incidence was seen as early as one year into the therapy.
The MICRO-HOPE study focused on these same outcomes in patients with DIABETES. An even greater effect on decreasing risk, especially of cardiovascular death, was observed:
Interestingly, the protective effect was independent of any blood-pressure lowering effect of the medication. Adverse effects of this medication can include headaches, dizziness, fatigue and dry cough. Kidney function must be monitored when starting this medication.
